A personalized plan of care with one or more of our members where your individual needs are explored in order to provide you and your loved ones with a personalized plan for your care
Your plan of care may include the completion of Wills, Advanced Directives, Powers of Attorney, legacy and memorial ideas, coordination of services such as physician appointments, assistance in seeking MAID and funeral planning

Our mission is to assure that no one in our community dies alone; there are grief therapists as well as private caregivers and hospice workers in our group

Leading with the heart in end‑of‑life care transforms caregiving from a task into a sacred act of presence. When caregivers approach each moment with empathy, they create space for dignity, peace, and authentic connection. Titles and credentials may open doors, but it is compassion that opens hearts — allowing both the caregiver and the person in transition to meet in shared humanity. In those quiet exchanges of listening, touch and understanding, healing happens not through medicine alone, but through love expressed in its purest form.

We educate each other in our areas of expertise; thus broadening knowledge of available resources and care both for ourselves as well as our clients and loved ones
